Step 1:
Choose Your Visit
Select the service that best fits your healthcare needs, including primary care, acute care, follow-up and medication management, medical weight management, or mental wellness and lifestyle care.
Step 2:
Complete Your Secure Intake
Complete your required medical history, questionnaires, consent forms, and other requested information electronically before your appointment.
Attend your scheduled secure telehealth appointment. Your provider will review your concerns, relevant medical history, medications, symptoms, and healthcare goals.
Step 3:
Meet With Your Provider
DISCLAIMER: Medication is prescribed only when appropriate and is billed separately by a licensed pharmacy.
Step 4:
Receive Your Personalized Care Plan
Based on your individual needs, your care plan may include treatment recommendations, prescriptions when clinically appropriate, laboratory testing, imaging recommendations, specialist referrals, lifestyle guidance, or follow-up care.
Some concerns may be addressed in a single visit, while ongoing conditions or treatment programs may require follow-up. Established patients can schedule follow-up visits for medication review, laboratory results, treatment adjustments, chronic condition management, mental wellness support, or continued weight-management care.
Step 5:
Follow-Up When Needed
What Makes the Process Different?
ClearPath is intentionally designed as a Nurse Practitioner-led telehealth practice focused on convenient access, individualized attention, and continuity of care. Treatment decisions are based on your clinical needs, medical history, and healthcare goals.
ClearPath telehealth services are intended for non-emergency conditions. If you are experiencing a medical emergency or severe symptoms, call 911 or go to the nearest emergency department.
Important Safety
& Expectations
Telehealth is appropriate for many healthcare needs, but not every condition can be safely evaluated or treated virtually. Your provider may recommend laboratory testing, imaging, specialist consultation, urgent care, emergency care, or an in-person evaluation when clinically necessary.
